Information Needed: Long-Term Care Survey

In Governor Cuomo's 2018 State of the State Address, he announced the launch of a Long-Term Care Planning Council to prepare for the emerging needs of New York State's growing aging population.  The charge of the Council is to examine New York's long-term care system.

The Council will analyze and identify existing service gaps in the long-term care system; determine cost-effective interventions; and prepare a strategic plan to meet the needs of New York's aging population over the next decade.

The New York State Office for the Aging and the NYS Department of Health have created the following survey to hear directly from you about what you are experiencing at the community level in terms of supporting the independence of older New Yorkers and their caregivers.  They are interested in learning what works well and should be expanded, as well as what gaps exist and what you propose are ways to fill those gaps.
